You will be able to choose the option to book Hootons, Bolton Wanderers Football Club, Unit 1B, Burnden Way, Horwich, Greater Manchester, BL66JW
If you are a health and social care worker requiring vaccination , please send an initial email to pharmacy.fal25@nhs.net and we will then contact you directly.
If you have not received a letter from the NHS or you are not a health and social care worker, please fill out the below form and we will get back to you when it is your turn to have your vaccination.

We anticipate moving down the cohort list set by JCVI, as the weeks roll by. You will be notified as soon as that information has been released.
